ALL REGISTRATION FORMS DUE BY AUGUST 19TH

Parents are required to have the following registration documents on file prior to students being admitted into their assigned classrooms:

• Birth certificate
• Immunization records (with latest updates)
• Physician's statement
• All forms not currently in the student file

Students will be admitted to their perspective classrooms AFTER all documentation is received by the Registrar.

 Updated immunizations are due  no later than October 31st. The Texas Department of State Health Services  report is Due November 7th.  If you have gotten a letter from me about your child's immunizations, please have your updated information to me no later than October 31st.
